We accept private pay and a variety of insurance plans, and we may also be able to bill as an Out of Network provider for certain insurance plans. Please contact gina@sfbraindoc.org for any insurance inquiries.
Every plan is different. Please ask your doctor’s office to fax us the front and back of your insurance card (415-627-9108). Or using your phone, take a photo of the front and back and email it to support@SFbraindoc.org, and please also include your Date of Birth. This will enable us to verify your benefits prior to your visit.
